I verify that I am a current member of National Association of Hispanic Nurses (NAHN), Transcultural Nursing Society (TNS) or Chi Eta Phi Sorority, Incorporated (CEP).
I understand that my tuition discount remains in effect only while I am a current member of the above partner organization, and will terminate should I become a non-member.
I understand that I am responsible for any fees incurred as a result of my default of this contract and that random checks of employment status may be done to verify the validity of this contract.
If I should become a non-member, I will notify the Office of Student and Alumni Affairs at Duquesne University School of Nursing within seven (7) working days of my change of status.
