Title IV Authorization Form

I authorize Duquesne University to apply my financial aid, including Title IV financial aid, to my student account for applicable registration charges including tuition, room and board, and other education related charges.  I further understand that this authorization will remain in effect until I rescind it.

*An invoice from the Computer Store must be submitted with this voucher before funds can be released.
